Policy

Booking

A $25 security deposit is required for all new clients, full sets, and online bookings. Pay with Venmo, Cash App, Apple Pay, or Zelle. Your deposit goes toward your total service — it’s non-refundable and non-transferable. First-time clients, please fill out the consultation and consent form before your appointment.

Cancellation & rescheduling

Life happens — we just ask for at least 24 hours notice. Less than 24 hours notice is a 50% charge of your scheduled service, and a new deposit is required to rebook.

Late arrivals

  • Up to 5 minutes late: no charge.
  • 5–10 minutes late: $5 late fee.
  • 10–15 minutes late: $10 late fee.
  • 15+ minutes late: appointment canceled — 50% cancellation fee and a new deposit to rebook. In some cases we can still see you with a $20 late fee.
  • No-shows: 100% of the service fee, plus a new deposit to rebook.

No-refund policy

All services are non-refundable. If you’re unhappy with your results or experience an allergic reaction, reach out within 48 hours and we’ll make it right with a complimentary fix or removal. We also recommend a patch test before your first appointment if you have sensitive skin.

Service requirements

  • Lash extensions: arrive with clean lashes. Visibly dirty lashes that can’t be cleaned may require a removal and full set rebook.
  • Fills: 40–50% retention on each eye. Below 40% and you’ll need a full set.
  • Mini fills: within 7 days of your last fill, 80–90% retention.
  • Foreign fills: additional $15. Unworkable previous sets may need removal and a full set.
  • Waxing: avoid AHA/BHA and vitamin A on the area for at least 7 days.
  • Avoid caffeine for 3 hours before lashes, and use the restroom before we start. Durations on the site are estimates.

Loyalty

Points live in our loyalty program. Log in on the booking site to see your balance. Accounts are non-transferable and cannot be stacked.

Photos & media

We may photograph your results for Instagram, the website, or marketing. If you’d prefer not to be photographed, say so before your appointment. We’ll never tag you without permission.

Informed consent

These are the same terms you initial on the consultation and consent form. Please read them before you sign.

  1. I understand that a full set of lash extensions can make the appearance of my own lashes about 30-50% thicker, and make my lashes appear 20-50% longer.
  2. I understand that the lash extension services have some inherent risk of irritation to the orbital eye areas, including the eye itself, and could result in stinging and burning, blurry vision and potential blindness should the adhesive enter the eye or should an allergic reaction occur.
  3. I understand that some irritation, itching or burning may occur on the skin if the bonding agent becomes into contact with it.
  4. I understand that this is a semi-permanent procedure, as my natural lashes will continue to grow and fall out normally, making touch-up or “refill” appointments necessary to maintain the original look achieved by replacing the lashes that have fallen out. Most clients require a fill appointment every 2-3 weeks.
  5. I understand that while every attempt will be made to provide me with the length and fullness I have chosen, my final result may not be what I initially envisioned due to different eye shapes.
  6. I understand that it is imperative that I disclose all of the information requested in the client profile/health history.
  7. I have cited all conditions and circumstanced regarding my health history, medications being taken, and any past reactions to products or medications.
  8. I understand that additional conditions could occur to be discovered during the procedure which could affect my ability to tolerate the procedure.
  9. I consent to “before and after” photographs for the purpose of documentation, potential advertising and promotional purposes. (optional)
  10. I understand that I am responsible for maintaining healthy lash hygiene in order to come with the best retention between 40-50%.

I understand that if I have any concerns, I will address these with my lash extension specialist. I give permission to my lash extension specialist to perform the lash extension procedure we have discussed, and will hold him/her and his/her staff harmless and nameless from any liability that may result from this treatment. I have accurately answered the questions above, including any known allergies, prescription drugs or products I am currently ingesting or using topically. I understand my lash extension specialist will take every precaution to minimize or eliminate negative reactions as much as possible. In the event I may have additional questions or concerns regarding my treatment, I will consult the lash extension specialist immediately. I agree this constitutes full disclosure, and that is supersedes any previous verbal or written disclosures. I certify that I have read, and fully understand, the above paragraphs and policies at https://www.lashesbylj.com/policy, and that I have had sufficient opportunity for discussion to have any questions answered. I understand the procedure and accept the risks. I do not hold Lashes by LJ and/or the lash extension specialist, Louajee Churchon, responsible for any of the conditions that were present, but not disclosed at the time of this procedure, which may be affected by the treatment performed.

If the client is under 18, a parent or legal guardian initials each statement and signs on their behalf.

I am the parent or legal guardian of this client, and I agree to the terms above on their behalf.

Right to refuse service

We reserve the right to refuse service. If we have concerns about lash health, we may recommend a break from extensions. Rude or disrespectful behavior means you’ll be asked to leave with no refund.
